If Isabelle never left Alec, what would have happened? Would he eventually have killed her?

Assuming that Isabelle never kills Alec herself, and that staying entails total loyalty, yes, he would have killed her when it became clear that his lung cancer was a terminal case. They would have gone out together in a murder-suicide.
